REGISTRATION
PROCESS
You must have:
1. Copy of Birth Certificate, or other proof of age.
2. A complete Medical Form dated after Jan. 1, 2008, signed by a Doctor certifying that the player is physically fit to play tackle football, by May 15.
3. A complete Player Registration Form (PDF), signed by the player and his/her parent/guardian.
4. A complete Liability Release Form (PDF), signed by the player and his/her parent/guardian.
5. A photograph, head and shoulders, NO HAT or CAP, 2"
x 2" or 2" x 3".
6. Copies of school report cards for all 2007-2008 school year grading periods.
6. Cash or check made out to "Palo Alto Knights" for $250.00 before April 1 or $300.00 on or after April 1.
NOTE: Fees are not refundable after August 1 (8/1/2008).
7. Check for $100.00 Volunteer Buy-out. (All parents are required to volunteer during the season. $100.00 buy-out will eliminate volunteer assignments) (PDF)
